To Make an Appointment:

If you are a new patient, please arrive 15 minutes before your appointment time to fill out paperwork or just print them above and fill them out at home.
Please note that we have changed our online scheduling software. If you previously registered to schedule online, you will need to re-register. Thank you for your understanding.
If you are interested in an appointment regarding bioidentical hormones, please do NOT schedule online. CALL THE OFFICE (474-5800) for information on hormone testing and specific paperwork that needs to be filled out prior to the consultation. Thank you!
Medical Questions:
For medical questions, you may either call or email us at the above email address. If you choose to use traditional email, please be advised that in doing so you accept the security risks of using non-secure email systems. If you desire secure email, look into Scrambler Mail (www.scramblermail.com) or let us know and we will send you a small file that allows us to email you securely. Please do not use email for anything that is time-sensitive; we will generally respond back within several days. Email consultations for new issues can be done for $25. Follow-up questions for established issues or reviewing test results are no charge.
