We participate in Medicare and many private insurance plans. If we do not participate with your particular insurance plan, we are happy to see you out of network. For those that do not have insurance, we do have a "paid at time of service" discount to help keep your cost reasonable.

FAQ's

What types of insurance do you accept?

We participate in Anthem Blue Cross/Blue Shield, Medical Mutual of Ohio, Aetna, Cigna, Humana, Great West,Ohio Health, United Healthcare and a few other smaller companies. If you have one of these you are responsible for your copay when you are seen and we will then bill your insurance company. After the claim has been processed and we receive the explanation of benefits, you will be responsible for paying any remaining balance within 10 days.

What if you don't participate in my insurance plan?

The way it works is that you pay for your visit at the time you are seen, and we give you a special insurance receipt (CMS 1500) that you use to file your own claim. Many private insurance companies will then reimburse you some or all of your fee according to your policy's out-of-network benefit, minus any copay or deductible amount.

What if I have Medicare or Medicaid?   

We are not open to new Medicare patients at this time. We do not participate in the Medicaid managed care programs (Molina and Care Source).

What if I have an HSA (health savings account)?                                                

We accept credit/debit cards for all payments, including those tied to your HSA. The payment comes right out of your health savings account. If you have met your high deductible health plan (HDHP) deductible, your payment to us should be fully reimbursed.

How much are office visits? 

Office visits vary depending on whether you are a new or established patient and how complex your visit it. They generally range from $60 - $140, which includes a 20% discount given for payment at time of service. This discount benefits those without insurance or with high-deductible plans.

How much are house calls?

House calls range from $100 - $145, which includes a 20% discount for payment at time of service.

We accept cash, check, or credit cards (Mastercard, Visa, Discover and American Express).
